How can I treat my really bad skin?

Well, on my upper back, on the shoulders and below the neck, I have a few spots, and pimples. I know, it sounds disgusting, which is why I want to know how I can get rid of it all. I've tried anti-bacterial soap and this gel. The only thing that has worked was the sudo cream, but then the spots came back again, and I have a few spots on my chest and it puts a damper on me wearing proper tops, I hide under jumpers. Please help, how can I get rid of them?
